Missed an exam

Extension Requests

If you missed your exam and want to request an extension form, please email [e-mail] within 10 days of the missed exam. If you do not contact us within 10 days of the missed exam, your exam fees are forfeit. You may also request the extension fee be waived if you missed the exam due to circumstances outside of your control.

FailSafe Applicants

FailSafe applicants, please be aware you are not eligible for extensions. However, if you would like to request that we waive the exam fee for your facility, you will need to contact [e-mail] with an explanation of and documentation for your extenuating circumstances that caused you to miss the exam within 10 days of the missed exam. If you do not contact us within 10 days of the missed exam, your facility will be charged the full exam fees.
